Our services span the Inner West and Eastern Suburbs to Western Sydney and the Southwest. We specialise in disability, mental health, and ageing, supporting people who require high intensity supports or who at risk of entering long-term institutional care. We do this by fostering positive relationships that build trust and confidence amongst people, enabling dreams to be shared and obstacles to be overcome

Behaviour Support
Behaviour Support is about supporting individual’s and their family, friends and other supports to reduce the occurrence and impact of behaviours that can get in the way of relationships, social contact and leisure opportunities. QHC's practitioners understand the purpose and role of behaviours of concern and work closely with you to set and achieve goals to improve your life.
Support Coordination
Our team will assist you to build the skills and knowledge to understand, implement and use your NDIS plan. We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.
Plan Management
A plan manager will help you manage and maximise your funds, so you can make the most of your funding and achieve your wellbeing goals! Our team supports many people to unlock their NDIS Plans and our focus is on providing support, information, and resources so our members can get the services and supports needed to live life well.
We focus on connection; with our clients, staff and local communities. We understand the difference a strong connection can make in people’s lives. It can be a common bond or shared interest that really cements a relationship. We work closely with you to get the right support match.
