About the Role


You will be responsible for assisting participants to understand and utilise their NDIS plan funding. 


You will do this by working closely with participants to identify services and supports that meet their need and enable them to work  owards their goals as outlined in their NDIS plan. Using a strength-based approach you will coordinate a range of supports across all areas of the participants life.  


You will offer the participants choice and control when

appointing providers and build the participants capacity to self-direct their supports. 


You will present options to the participant and engage service providers as directed by participant, you will oversee the continued supports and manage any issues that may arise between providers and participants. 


You will assist the participant in navigating all areas of their plan. You will also build on community and non-funded supports to maximise supports in the participants life. 


You will liaise with government and non-government bodies, obtain quotes, oversee budgets, follow up on reports and advise NDIA of any changes to the participants circumstances and submit progress reports as required.


Skills and Experience


  • Relevant tertiary qualifications or certificate in relevant field e.g. Social work, community services or allied health.

  • Experience in providing services to NDIS participants as a Support Coordinator or Case management experience

  • Experience in reading, understanding, and implementing NDIS plans

  • Experience in connecting clients to services under a client direct care model

  • Excellent interpersonal and communication skills and a strong commitment to customer service

  • Ability to manage a diverse participant portfolio

  • Ability to maintain positive relationships with providers & community agencies

  • Strong customer service skills with demonstrated ability to develop and maintain relationships with people with disability and/ or their families and support network and service providers

  • Resilience and competence in high level interpersonal communication, negotiation and conflict resolution skills

  • Sound time management skills and demonstrated ability to work with minimal supervision

  • Demonstrated ability to make complex judgements, prioritise tasks and take initiative

  • Strong written communication skills with the ability to write high quality reports

  • Current driver's license and willingness to travel to clients where required

  • Previous experience in a billable environment highly desirable


Successful candidates will be required to clear probationary checks including National Criminal History Record Check & Working with Children Check & NDIS worker screening.
